About Me

My name is Sara L. Zulski, R.N. My skills and areas of expertise include administration, negotiation, nursing, and case management. My professional title, previously was Case Manager at Northeast Michigan Community Service Agency, Inc. (NEMCSA), which specializes in Case Management. Currently I am working with Avalon Consulting Inc, performing Humana assessments to help those that are at risk for future hospitalizations understand their disease process and offer community resources to them.

One of the main initiatives I have been intricately involved with is the Nursing Facility Transition Program, to which consumers have the right to make decisions about their own lives, their well-being, and the right to choose where they live and receive care/assistance. This program empowers an individual and places control back in their hands regarding many aspects of their lives; allowing for the “most integrated setting” possible. Federal studies have shown that a person placed in a nursing home for one month totals $7,000. The Nursing Facility Transition Program brings that cost down to no more than $3,000. As a Case Manager, I go out into the community, research the various services that are available and provide this information to clients. These cases are managed for a year, to ensure a smooth transition from nursing home to a residency they feel comfortable with.

One of my goals is to provide a leadership and management role with regards to holistic medicine. On the personal side, I want to continue being a great example to my children that women can succeed in everything they set their minds to.

I am currently involved with the American Nurses Association, the Commission for Case Management Certification (CCMC) and the American Nurses Credentialing Center (ANCC).

My hobbies and interests include many outdoor activities. I love spending time with my family. I am drawn to and passionate about charitable organizations such as St. Jude Children's Research Hospital.
